Below are school & health forms due back this summer. If you have any issues with downloading or signing forms, please email communications@thewindwardschool.org. Please submit your school forms to your student's main desk (email addresses are listed below), and please submit your health forms to Schooldoc.com.

Health Forms

In our continuous efforts to provide the best possible care for our students, The Windward School is partnering with SchoolDoc.com. Every summer, incoming and returning families will receive an email invite from SchoolDoc.com to access your account. Health forms are to be uploaded to SchoolDoc.com before the first day of school.

SchoolDoc.com is a secure, encrypted, and password-protected electronic health record system that will help us consolidate and integrate student health information into a centralized and confidential location. It will give our nurses instant access to student health information, a key component in providing quality care.

Below are links to the health forms. Once completed, and you have access to SchoolDoc.com, they can be scanned or photographed and uploaded to your SchoolDoc.com account. The forms are also available in SchoolDoc.com; however, we wanted to make sure you had early access to them to complete them at your convenience.

Forms to be filled out and uploaded through your SchoolDoc.com account by August 17, 2018.

A current health appraisal form is required for ALL NEW students, all students entering Grades 1, 3, 5, 7 & 9, and ANY student in grades 5–9 wishing to participate in any after-school athletic program. An annual exam is recommended for students in other grades. Current physicals are due by August 17, 2018.

New York State Public Health Law requires all students be immunized according to mandated guidelines. Regardless of the state you reside in, The Windward School must receive proof of immunization for all required doses (see charts below). Proof of immunization must be any 1 of the 3 items listed below:

    • An immunization certificate signed by your health care provider
    • Immunization Registry report (NYSIIS or CIR from NYC) from your health care provider or your county health department
    • A blood test (titer) lab report that proves your child is immune to the diseases
      • For varicella (chicken pox), a note from your health care provider (MD, NP, PA), which says your child had the disease, is also acceptable.

Manhattan Lower School Forms

Manhattan Lower School forms for the 2018-2019 school year are listed below. All forms may be filled out on your computer. Once you have filled a form out, save it, and attach it to an email. Please submit them via email to mlsmaindesk@thewindwardschool.org, or you may submit them by paper copy to the Manhattan campus front desk.

The following forms are only due when they are needed:

The following forms were sent out with your school contract or in May 2018. You will be contacted individually by your student's division if they do not have one on file. There is no need to fill out another one.

Manhattan Middle School Forms

Manhattan Middle School forms for the 2018-2019 school year are listed below. All forms may be filled out on your computer. Once you have filled a form out, save it, and attach it to an email. Please submit them via email to mmsmaindesk@thewindwardschool.org by Wednesday, August 1, 2018.
