Initial Interest Survey for MMS Athletics Program

This form is for Manhattan Middle School families only. This is a survey to gauge your child's interest in sports for the 2019-2020 school year. If you think your child would like to participate in any of the following sports, please check the appropriate box. When each season begins, a formal sign-up will be e-mailed and available online. When the permission slip and update card are completed, we will put your child on the permanent roster.

 


Winter 2019/20 Sports

All winter sports begin the 3rd week of November, except for instructional basketball which begins the first Monday in December.

A FULL COMMITMENT IS REQUIRED TO PLAY ON INTERSCHOLASTIC TEAMS.

Club and interscholastic teams compete against other schools while intramural and instructional athletics work on the fundamentals of the sport.

Spring 2020 Sports

Physical Examination Requirement for Interscholastic Sports

NEW YORK STATE PUBLIC HEALTH LAW REQUIRES ANY STUDENT PARTICIPATING IN AN INTERSCHOLASTIC SPORT HAVE A CURRENT PHYSICAL EXAM.A current physical exam is one that is administered not more than 12 months prior to the commencement of the school year. The examination for athletes wishing to participate in any interscholastic sport must be administeredless than one calendar year from the date the sport begins. Any injury or significant illness sustained after the date of examination nullifies the physician’s clearance and will require further medical documentation in order to be cleared for sports participation.

The school physician will administer physical exams for those students who choose not to utilize their own physician. Exams will be completed on Wednesday, August 28, 2019. Please call to schedule an appointment during the week of August 19. All appointments must be scheduled by Friday, August 23.

IMPORTANT INFORMATION:

  • The school physician does not perform laboratory work, clear injuries or give immunizations.If your child needs vaccines, a clearance letter or lab work, please see your own physician.
  • Epi-pens and inhalers for students with life threatening allergies, students with reactive airway issues or asthma must be brought in with the completed Administration of Medication and Allergy Form prior to the first practice.These medications are carried in the emergency kit which goes to all practices and games.
  • If your child is injured at the time of the physical the school physician will not clear your child to participate.
  • Any consecutive absences from school of 3 days or more will require a new clearance letter from the physician. Any injury which prevents your child from participating in physical education class or sports will also require a medical clearance letter from the child’s physician.
